What Happened and What Information Was Involved:

Your Patient Advisor is a service of Captify Health which provides patient management services for colon exams

Your Patient Advisor recently announced that in March of 2021, Your Patient Advisor was contacted regarding fraudulent use of consumer credit cards potentially related to their payment card environment. On October 13, 2022, Your Patient Advisor’s investigation determined that their website was compromised and some information related to certain individuals, including their personal information may have been exposed. The impacted information may have included:

• Full names,
• Addresses,
• Dates of birth,
• Payment card number,
• Expiration date, and
• Security codes.
